3 Easy Steps

How To Use

Step 1

Keep your ID card with you at all times to easily present it at provider locations. You must present your ID card at the time of service to receive a discount. To access your ID card, log into your MyLoomis portal. You can print a copy and visit your portal at any time.

Step 2

To locate and verify participating providers in your area, simply call Member Services at (800) 290-0523 or use the PROVIDER SEARCH tool. When calling to schedule an appointment, please refer to your membership card for proper network identification.

Step 3

You are responsible for paying the provider the entire discounted fee at the time the service is provided.
